1. If you have an existing Errors & Omissions insurance policy please download this certification form.  Fill in the top and have your insurance carrier or company complete, sign and certify the details about your existing E&O/Legal Liability/Freight Services policy.  We don’t need the premium cost or many other details – we just need to know you are covered, the maximum amounts of coverage, the deductible amounts and the expiration date.  Send the completed form to WCA Family’s in-house attorney Michael McMullen (fax: +1 619 330 1886 or via email mmcmullen@wcafamily.com ) and upon approval, the Risk Managed logo will be placed next to your company on our website. Please ensure the insurance company representative or agent certifies your coverage and signs the form.

Our specialists can assist you with your insurance matters and make sure that you have appropriate cover or provide you with cover via our unique FSL policy.
